Med or Nursing students help - BP reading

Sorry, I couldn't find a health-related forum so I thought to post here.


I have a Certificate of Health for a university in Japan that my family dr filled out.

However, it looks like he left the blood pressure reading half empty.

The form says ____________mm/Hg ~_________mm/Hg.

He only filled in the first part with, ie, 125/84 mm/Hg.


What was supposed to go in the second field? Can anyone help?

It looks like he put the whole reading in the first space.

From how you lay it out, put the 125 in the first space (systolic BP) and put the 84 in the second space (diastolic BP).
