Yorku: Faculty of Science and Engineering

I'm going to transfer into the Faculty of Science and Engineering, and I filled out the Transfer Form a couple of seconds ago...However, the back of the form says:

"Applicants to the Faculty of Science and Engineering must refer to the supplementary sheet for additional information on documentation and admissions requirements to the Faculty. The supplementary sheet is available from Science Academic Services, 352 Lumbers Building."

I have no idea what this "additional information on documentation and admissions requirements" could possibly be, and no way in hell am I making the grueling trip to Keele campus for it. I haven't found the sheet online, either.

Does anyone know what else is needed to transfer to this Faculty, beyond the Transfer Form?

"The SAS office is located in 352 Lumbers Building and is open 10:00 a.m. to 4:00 p.m. Monday to Friday. You can receive telephone assistance between 8:30 a.m. and 4:30 p.m. (Every Friday during the summer, service is only available until 3:30 p.m.) at (416) 736-5085."
