So, here's a question. If I mail a package of a ensure (meal replacement, manufactured in US) from the states to Toronto, and I mark it as a gift, how much duty, PST, GST would the recipient have to pay?

No duty if manufactured in the US
Canada Customs may accept it as a gift and you pay no PST, GST or maybe not ... but you should expect to pay GST and PST.
Did you ship it USPS ... then $5 brokerage charge (USPS hands off to Canada Post).
Did you ship it UPS Cheap ... $0 to $75 for brokerage charges, depending on the value of the goods, (details here: http://www.ups.com/content/ca/en/shipping/cost/zones/customs_clearance.html)
Some other courier ... phone but probably in line with UPS

Ensure can be bought in Canada -- believe it or not!
You'll end up paying more for postage than it would be to buy in Canada.
Also, in Canada, we have a product called Boost. Nutritionally, it's the same or better than Ensure, but the taste is much better than Ensure.
