X360 HD-DVD - free movie offer.. dumb question

On the form to mail in for the five free HD-DVDs, one of the fields to fill in is "HD DVD product model number". Now, on the barcode of the box.. I don't see anything labelled product model number (I see the serial number, a "team" number, "lot" number, "wo" number.. ). What value should I be putting in here?

Perhaps this is a dumb question, but I don't want to see 5 HD-DVD movies go down the drain because of some stupid technicality. :D

There should be a customer service number on that form, I had similar question when i was mailing for 5 bluray offer for PS3. Better ask directly people that you are mailing it to, that way you can also blame it on them :D
