I specifically asked my plumber not to put any in but his crew installed their cheap entry level ones anyway.

Besides being off center, they are about 53" high off the floor which just seems ridiculously high. They did this in all 3 bathrooms and the toilet roll holders have the same problem.

So now I'm looking at removing them, and patching the big holes caused by their anchor bolts then repriming and painting.

Is there a standard height for towel bars? And should I be asking the plumber to take responsibility for the problem and work to remove and repair it?

Whatever height is most comfortable for you considering your height. If you are remodeling a home to sell, I would place it at around 4 feet. Just make sure that a large bath towel will not drag on the ground.

Standard adult towel bar height is indeed 48". For kids, 36" is considered standard. However, you should take into account placement of switches, plugs and HVAC vents. Of course, these heights can vary to taste, or if you want to "line up" with something else in the room. And, if you're rennovating, don't forget to put blocking in the walls for a solid mount through the drywall or tile.

I would also be speaking with the person that did this if I had asked them not to do it he is responsible for his crew, although it wouldn't be a big deal to fix it yourself (if you already have the materials) but making him aware at the very least is the way to go.
