The cons of running lean are huge - you can destroy your cylinder head, valves and/or pistons in no time.
There are some ways you can do lean burn safely, but it sure is not easy - manufacturers put billions into developing GDI (gasoline direct injection) in order to be able to lean out the mixture by about 10% under SOME driving conditions, by being able to concentrate the mixture around the spark plug. The technology is still too expensive in Canada compared to the price of fuel. But i did drive a nice Mitsubishi Galant in Sweden that had a GDI engine. As far as I remember, in Canada only the BMW 760iL has GDI, but I'm not sure if that is the only car with GDI available here.

Some non-DI engines can run lean as well, but there aren't many in North America. The lean burn Honda Insight comes to mind. I don't know if the engine has to be specially designed for that or not. Running lean will probably increase NOx emissions.

For something cheap and decent, find a old Apex'i SAFC

you will most likely only see improvement changing the A/F Ratio if you did any I/E/H modification. or maybe changed the vtec point for honda engine. but i don't think 91 accord has vtec. i think it was the F22A block on 91 accord.

you might wanna look at ECU reflash or something like that instead =)

Yep! There's an old school one, then a newer black one. Change your A/F on the go from your driver's seat!

Get it tuned professionally. Teknotik does ECU tuning through a dyno and they are well accomplished. Look them up here: http://www.teknotik.com/contact.php
