Performance Appraisal

I'm due for a performance appraisal and my manager requires me first to do a self performance review before he does the review himself. Anybody have tips and do's and don't on doing a self-performance review so it won't bite me in the end during the appraisal? This is first time I ever came across this. Thanks in advance.

Focus on your accomplishments/achievements during your tenture there. What did you do to drive value to your company and or to your position? Also, what are some key focus areas that you could improve in skill or experience wise? Ensure you have thought of some possible training/courses that could help you so that you can include it in your development plan section of the PR.
