I have a Canadian American Express, how do I get a US - American Express?

You can call AmEx Canada and tell them you want/need a US AmEx card. If you meet the qualifications for it (ie: valid US address, valid US social security number, etc.) AmEx Canada will help make arrangements for a card with AmEx in the US on your behalf. But you have to meet the qualifications for the US card like anyone else. You also have to have good credit in Canada. The benefit is that AmEx Canada guarantees to AmEx in the US that even though you have no credit history in the US you can be trusted with a US card.

Then you're pretty much out of luck applying for a US-issued card. According to most US*issuers, you must provide a valid social security number with your application under federal US law. I don't know what the law is, or what its specifics are, but any card application in the US has the social security number as a mandatory field.
If you just want a US dollar card (and not a card issued in the US) in order to do transactions in US dollars for whatever reason, you can take out a US dollar card at most Canadian banks. However, paying the bills on those cards can a hassle.
American Express does offer an International US Dollar card meant for non-US citizens who need to, or want to transact in US dollars rather than their home currency, but it is only available outside of North America. You can call AmEx in the US and ask them about it as depending on your specific circumstances and needs they may be able to help you out.
