Lost My Passport!!!

Okay Guys I am going to need some help here, here is my deal I lost my passport (2nd time/first time was in 2001) so I know I need to report to the police as lost/stolen, after that I have to go my doctor/sponsor(Person that knows you 5 years or more). So what I am wondering 1) Has anything changed in the last couple of years and Do I need anything else and 2) Where could I find the form for my sponsor to sign in PDF format?

go to the passport office off hwy 10 near bovaird and get the forms from there its behind the new Keg there

One of my friends had his Passport and citizenship card stolen recently and he said he got both replaced in 7 days even though the passport office said it could take months...
