I noticed on the Staples rebate form that the 0403 number is linked to the US model and the Canadian model is 0215. The Canadian model is what I had received from Staples.
I contacted Logitech about the rebate as well as the Rebate Status guys, and they said flat out that if the part number does not match exactly, your rebate will not be honoured. They suggested I contact Staples and have them issue new forms with the proper part number (931179-0215) as the forms they are currently posting are incorrect.
I called Staples to tell them to:
A) Update the form on the web site with the proper part number (931179-0215)
B) Give me the $10 rebate up front as they were neglegent for misleading customers to believe they will get a rebate when in fact they won't. They have not posted any retractions or updated their ads to post this and they should provide a quick remedy to the customer for their wrongdoing.
I am awaiting the outcome... stay tuned!

Just got a call from Staples that they will be updating their rebate forms with the correct part number (931179-0215).
Folks, before you mail in the forms, make sure that the part numbers match EXACTLY or else you won't get the $10.
Hope this helps.

The form generated by the cash register has both the correct UPC code: 097855023056

Not talking about the upc number, the part number is different.

Thanks for the post. I had my staples-supplied rebate form with the incorrect part number all filled out and ready to mail, which thankfully I didn't because I don't have any stamps. I thought I had read on the other post in 'hot deals' that a staples manager said the mismatched numbers wouldn't be a problem.
Hope they update the rebate form or do something to fix this potential problem soon.

Well obviously they will say it will not be a problem or else they would be admitting a fault.
They will talk to the person who does the web forms to make the update, either today (if he is still in) or tomorrow
MAKE SURE YOU GET THE RIGHT REBATE FORM BEFORE MAILING IT OR ELSE YOUR SCREWED!

No but as with any rebate, make sure your part numbers and codes match up or else you won't get the $$.

It looks like the web rebate form (http://staples.rebatestatus.ca/tearpad/057489_03312005.pdf) has been updated so the part number is now 931179 (they truncated the last 4 digits.
So I just tore apart my original rebate envelope and now I'm confused. My receipt only has the product UPC code (which matches the UPC from the box 097855023056), and no part number on it. I don't see the part number (American or Canadian) anywhere? I don't see why they wouldn't honour this rebate with either form.

You should also post a link to this thread somewhere along the 'hot deals' threads relating to the mouse; though OT forums was the correct one to start with, some may miss it here, so just an idea [to post it over there as well.] Thanks for taking the time to post of this error that could have affected so many. :)

That was the reason why I posted this in the HOT DEALS section because in an OT area, it could be missed. Guess the Admin didn't see my logic
