To take part in this offer, you will need to complete this rebate form (PDF) and mail it in with your receipt -- make sure you circle the $40 or more in participating products. This offer is valid for receipts dated from February 24-April 15, 2012, and the form must be received by April 30, 2012 in order to be eligible.
